Guidelines for Delegating Limited Authority to

STATUTORY REFERENCE:
A.R.S. §32-1436(C)
may return the license renewal fee on
special request.
CIRCUMSTANCES OF USE:
The Executive Director may grant license fee refunds:
· If it is determined the applicant does not currently meet
the statutory requirements for licensure and requests a refund in
writing within two weeks of notification.
· If the applicant requests to have the application withdrawn
within two weeks of the Board's receipt of application.
· If a physician who has paid the renewal fee dies before
the late fees would be assessed and the family requests a refund.
