How do I request information from the Board?
 

You can send a request via fax, postal mail, telephone, in-person or on our website. Once we receive your request, you will be sent and must complete a "request for information form." Once we receive this form from you, we will process your request within ten days.

 

What information is available from the Board?
 

License file information, board disciplinary orders, transcripts from board meetings, formal interview and hearing transcripts, copies of dismissal and advisory letters and other disciplinary actions (letters of reprimand, censures, suspensions, probation and revocations), board meeting agendas and minutes.

 

Is there a charge for documents?
 

Yes, for copies.

 

What information is available on the Physician Profile on your website?
 

The physician's full name, office address, city, state, zip and phone, educational background including medical school, location of residency and internship, specialty, year license obtained and date of renewal license status, all disciplinary and non-disciplinary actions and open investigations. The allegation of the open investigation is not public information until the case has been heard and decided by the board.

 

Why is the allegation of an open investigation not public information?
 

Until the Board finds the allegation has truth and is based in fact, and a decision concerning disciplinary or non-disciplinary action has been made, the nature of the allegation is confidential.

 

Why are the details, records and information of the investigation confidential?
 

Arizona state law specifies that the Board's investigative files are confidential. The Board's intention is to protect the complainant and the person named in the complaint; therefore, confidentiality is strictly maintained of all the patient's records, any information from which a patient or his family might be identified, or information or records received by the Board.
